Supporting documents for Visa Application:

We will prepare the documents at you request for Visa application.

Please follow the instructions below.

Download and complete "Visa Support Information Form" below.

Send the completed form and a scan of your passport's identification
page (photo page) by email to tsubomi@mail.kek.jp.

Once we receive the form and a scan of your passport, a confirmation
email will be sent to you.

When we prepared the documents, we will send you the original
documents by air mail.

The deadline for submitting "Visa Supporting Information Form" is
November 30, 2015. (JST)
